Payment Forms

In order to streamline and simplify payments to vendors and teachers and for refunds, PGHW is switching to an online payment program.

We ask all of members and teachers to fill out the following forms. All form information is sent directly to our treasurer and no forms are kept online.

You must also fill out a W-9!! 

For your convenience, we have also attached a “fillable pdf” W9 form we require for anyone teaching, speaking or performing other services for PGHW. Please fill out the form, save it to your computer and email it to treasurer@pghw.org. Your signature is implied. 

 

This form is to be filled out by anyone requiring payment for a workshop, monthly program or a reimbursement. (prior approval by guild President required)

PGHW is no longer cutting physical checks except in rare emergencies – electronic delivery is a convenience for everyone involved.  PGHW’s Treasurer will send your payment/ reimbursement / refund via ACH directly to your bank account.  Your information will be kept completely confidential and never shared. Our Treasurer will send an email when payment is on its way so you know to look out for it.
